PreParticipation Physical Form 2021
Everyone will need a current physical. If your physical is still current then you will need to complete 1st few pages with the Covid/History questions on the new PreParticipation form (attached). If your physical has expired then your new physical needs to be on the new form and you will need to complete the Covid/History questions on the new PreParticipation form.

Gfeller-Waller Concussion Form 2021
This form needs to be completed and initialed by both the student-athlete and the parent/guardian. Make sure that it is dated July 1, 2021 or later.
